Along with the coagulation factors (II, VII, IX, and X), the anticoagulant proteins C and S are also vitamin K-dependent, and therefore affected by warfarin, which is a vitamin K analog. Proteins C and S have shorter half-lives than the coagulation factors, and therefore a faster turnover in the body. Thus, for a brief window of approximately 3 days after the initiation of warfarin therapy, the body has normal function of its coagulation factors, but loss of anticoagulant proteins C and S, rendering the patient hypercoagulable.
Negative consequences of this are avoided by administering heparin with warfarin for the first 5 days.
